State officials probe death of 3-year-old who fell from 2nd-story window

State child welfare officials are investigating after a 3-year-old boy died Wednesday evening, one day after falling from a second-story window on the Northwest Side.
Mohammed Alyamani fell from the window about 1:25 p.m. Tuesday at his home in the 3700 block of North Kostner Avenue, according to police and the Cook County Medical Examiner’s office.
The boy was taken to Advocate Lutheran General Hospital in Park Ridge, where he was pronounced dead at 7:30 p.m. Wednesday, the medical examiner’s office said. An autopsy is scheduled for Thursday.
The state Department of Children and Family Services is investigating the boy’s mother over a supervision issue, according to spokeswoman Veronica Resa.
Four other children in the home have been placed with a relative under a DCFS-monitored safety plan, Resa said.
Area North detectives are also conducting a death investigation.
